You can use our Request a statement of reasons form to ask for a written explanation of why the Tribunal made a decision.
Ask for another Tribunal hearing
You can use our Request another Tribunal hearing form to ask for a hearing to have your treatment order cancelled (revoked).
It usually takes about 8 to 10 weeks from when you ask for another hearing to when your hearing will be held. We will call you if we haven't been able to schedule a hearing after 8 weeks.
You can also apply to VCAT for a review. Applications to VCAT must be within 20 business days of the Tribunal's decision or receiving a statement of reasons.
What I want to tell the Tribunal form
You can use this form to prepare for your Tribunal hearing by writing down what you want to tell the Tribunal. You can use this as notes at your hearing or submit it to the Tribunal.
Application to the Mental Health Tribunal (MHWA 114)
You can use this form to:
- request another Tribunal hearing to have their Treatment Order cancelled (revoked)
- make an application against being transferred to another designated mental health service
- make an application against a refusal to grant leave of absence (if you are a security patient)
