We are conducting a survey to learn about patients and carer's recent experience of Tribunal hearings to help us improve.

This survey is for patients, consumers, carers, family members, friends or nominated persons aged 18 and older who were invited to a hearing held in 1 October 2019

The survey is entirely voluntary and anonymous. You can skip any questions you don’t want to answer.

If you have already completed this survey in 2019, please do not complete it again unless you have had another hearing.

Privacy information

The Tribunal is conducting the survey. Responses to the survey will be used to review how the Tribunal conducts and manages hearings. An independent researcher working for the Tribunal will analyse the survey data. The Tribunal will share the findings of the survey on our website and with stakeholders.

Your individual responses will not be identifiable as the survey is anonymous. The Tribunal will securely store and dispose of all individual survey responses by September 2020.
